A Medway patient has become the first to undergo same-day surgery on both hips within six months, marking a significant milestone in Medway NHS Foundation Trust’s development of same-day surgery services.
Sharon Clare, 60, from Rochester, had lived with debilitating hip pain for years before being referred to the orthopaedic team at Medway Maritime Hospital. Following a referral by her GP and an X-ray, she was assessed by a physiotherapist and then met Trauma and Orthopaedic Consultant Mr Rajesh Bawale, who diagnosed severe arthritis in both hips.
“Even walking or getting up from a chair was painful,” Sharon said. “I wasn’t functioning as I should have been. I was just pushing through the pain.”
In May 2024, Sharon had her first same-day hip replacement. Just six months later, in November, she returned for the second. On both occasions, she was back home that same evening.

Before her first surgery, Sharon shared: “I was petrified. I thought I wouldn’t be able to get on my feet at all.”
“After the first surgery, it was the best thing ever. I was up and walking just hours later. I had freedom in my leg again. The staff were lovely, keeping an eye on me the whole time.”
Sharon’s second surgery was equally successful. She was discharged in time to catch Big Brother on TV that evening.
“I’m now back to walking long distances, driving, climbing ladders – even getting down to the floor and back up again. It’s truly life-changing,” she said.
Sharon’s case was a first for both the hospital and Mr Rajesh Bawale. She was Medway’s first patient to undergo bilateral hip replacements as same-day surgeries, and his first same-day hip patient overall.
Mr Rajesh Bawale explained: “Same-day hip arthroplasty was introduced here last year and Sharon was a perfect candidate. Patient selection is key to success as not all patients are suitable for day surgery.”
Traditionally, patients would stay in hospital for several days after hip replacement surgery. Ongoing improvements in surgical planning and performance, and recovery support mean that more patients can have their operation and go home the same day.
A key part of this is the SMART team (Surgical, Medical and Acute Recovery Team), who support patients recovering at home through nursing, therapy, and remote monitoring.
Mr Rajesh Bawale said: “It’s a huge team effort – from the SMART team to the physiotherapists, X-ray staff, nurses, and surgeons. Everyone plays an important role.”
Sharon received a follow-up call from Mr Rajesh Bawale on the evening of each surgery, along with regular check-ins from the SMART team to ensure she was recovering well.

“The home environment helps a lot mentally,” said Sharon. “You’re more comfortable, more relaxed. The communication from the team was amazing. I never felt alone.”
Mr Rajesh Bawale added: “If a patient is nervous, we want them to know we’re here for them. Communication is key. No patient should ever feel unsupported at home.”
Reflecting on her experience, Sharon said: “Mr Bawale and his team gave me my life back. If you’re eligible for this type of surgery, I would 100 percent recommend it. Yes, it sounds scary, but the support is there every step of the way. It’s changed everything.”
Looking ahead, Mr Rajesh Bawale and the team are keen to expand the same-day surgery service.
“We’re proud of what we’ve achieved so far, and feedback from patients like Sharon helps us improve even further.”
Since launching the service, the Trust has performed 24 hip and knee joint replacement day case surgeries, with a goal of reaching 50 by the end of 2025.
